I see my app is deleted because i need other format,

can anyone tell me how to get the rigt format??

u dont need a format just answwer the questions posted on the mod application forum it says :
MOST READ BEFORE APPLYING.

i mean by format as in the questions answered not the colour and stuff juist the questions answered
